Out of State Permission Form Players

(This Form may also be used to request a TOURNAMENT GUEST PLAYER.)

 

This form must be completed by all players requesting permission to play with a state association other than the state in which they reside as well as by any player moving from one state association to another during the Seasonal Year.  It is the responsibility of the player and parent/guardian to submit the form to the HOME STATE OFFICE (OYSAN).

 

For those who wish to guest play but are not currently registered with a team, please use the same form.  A player pass will need to be generated so your child can be OYSAN approved and eligible to guest play. (You may use the photo upload section on the form for the player photo portion of the player pass.)

 

 

US Youth Soccer Rule 201 requires that a youth player register each seasonal year in the State Association in which he/she resides with his/her parent(s) /guardian(s).  A youth player wishing to participate with a team from a state association other than the state in which they reside must receive written permission from both state associations prior to participation.

 

• The player must register and pay any appropriate fee(s) in the state in which they reside.
• OYSAN must complete the Home section of this form. The "Accepting State Association" will also send approval to OYSAN.
• Registration/Out of state fees:  Travel players: $25; Recreational players; $15.  Out of state fees for those already registered with OYSAN for the seasonal year:  $10
