Out of State Request Form for TEAMS

This form must be completed by all teams requesting permission to participate in league play within a state association other than the state in which they reside.  It is the responsibility of the team and coach/manager to submit the form to the HOME STATE OFFICE (OYSAN). 

US Youth Soccer Rule 201 requires that a youth player register each seasonal year in the State Association in which he/she resides with his/her parent(s) /guardian(s).

• The team must register and pay any appropriate fee(s) in the state in which they reside.
• OYSAN must complete the Home section of this form.
• Accepting State Association must complete the Accepted section of this form.
• Permission to play is valid for seasonal year applied for and must be obtained every seasonal year.
• Cost is $25.00 per TEAM
