Instructions for Player Release/Transfer Form (update 9/27/12)
This form is to be used when a player is requesting to be released from a team and/or when a player is requesting to be transferred to a new team within the same seasonal year.

The player release portion of this form must be filled out in its entirety before a transfer can be honored.

The player’s pass from the team that he/she is being released from must be turned in to the issuing league. A player release is to not be honored until the pass and completed player release form has been turned in to the appropriate league.

A player may apply for a player transfer once they have been granted a player release.

If the player is requesting a transfer, the completed player release must accompany the player transfer request along with the new team registration.

All player transfers must be approved by the player’s new team member organization before OYSAN will consider approving the transfer.

Transfers from one club to another within the same league must be completed by the league registrar.

Transfers from one club to another playing in a different league must be completed by OYSAN.

It is the responsibility of the new team member’s organization to verify that the player has been released from the existing team and is eligible for a transfer.

All signatures and fields on the player release/transfer form must be completed before a player transfer can be issued.
*Registrars should not “drop” a player from a roster if the player will be transferring to a new team.*

OYSAN will not process incomplete player release/transfer forms.
