Out of State Permission for TEAMS
This form must be completed by all teams requesting permission to participate in league play within a state association other than the state in which they reside. It is the responsibility of the team and coach/manager to submit the form to the HOME STATE OFFICE (OYSAN).
***THIS IS NOT FOR TEAMS TRYING TO OBATIN A TRAVEL PERMIT FOR TRAVELING OUT OF OHIO NORTH FOR TOURNAMENTS. For tournament travel permits, please visit www.thetournamentcenter.com.
US Youth Soccer Rule 201 requires that a youth player register each seasonal year in the State Association in which he/she resides with his/her parent(s) /guardian(s).

The team must register and pay any appropriate fee(s) in the state in which they reside.

OYSAN must complete the Home section of this form.

Accepting State Association must complete the Accepted section of this form.

Permission to play is valid for seasonal year applied for and must be obtained every seasonal year.

Cost is $25.00 per TEAM
Note: You may submit the form online, or print and fax to the OYSAN office if preferred.
